Social Prescribing Link Worker

Our Social Prescribers work alongside Primary Care providers and other healthcare professionals to help patients connect with non-medical community services and resources to improve their health and wellbeing. They are employed by the PCN to help patients identify issues affecting their health, such as social isolation or lack of activity, and make referrals to local community resources, such as community groups, sports clubs, or voluntary organisations.

To find out if you can be supported by this service, please ask a receptionist at your GP practice to book you an appointment.

How we can support you

Your GP, nurse or healthcare professional can refer you to a link worker.  They will speak to you on the phone or meet face to face to:

  • Discuss your problems
  • Find out what is important to your wellbeing
  • Help you access support, activities, or services and make best use of them
