Our Clinical Team
Care Coordinator
Our Care Coordinator work collaboratively with patients, families, and healthcare professionals to coordinate and manage care for patients with complex medical needs. This may include identifying and coordinating services such as medical care, social care, rehabilitation, and mental health services, as well as helping patients navigate the healthcare system.
Care Coordinators may also provide educational support to patients and their families, develop care plans, monitor patient progress, and communicate with other healthcare providers involved in the patients care. The goal of the NHS Care Coordinator is to improve patients’ overall health outcomes, quality of life, and healthcare experiences.
